What to Expect During Your First Therapy Session (Intake Appointment)

Especially for those of us whose identities have been marginalized—whether by racism, heteronormativity, or systemic oppression—it can take immense courage to take that first step.


If you’re considering therapy, you may have many questions or uncertainties. We believe that knowing what to expect can help ease your mind and empower you to begin this journey with more confidence. Here, we’ll walk you through what typically happens during your first session (intake appointment) at our practice, so you feel prepared and supported.

Understanding the Intake Process: Your First Appointment

An intake appointment provides an opportunity to meet with a culturally responsive, trauma-informed mental health professional in a safe, affirming space. At our Center, intake appointments are designed to help you:

  • Share what’s bringing you in

  • Clarify your needs and goals

  • Learn about potential approaches to care, including individual therapy, relationship therapy, or referrals for specialized support


Your first consultation generally lasts about 50–60 minutes. During this time, your therapist will ask questions to better understand your concerns, personal history, identities, and the contexts that shape your life.


We prioritize creating a non-judgmental environment where you can be fully yourself—whether you are navigating anxiety, trauma, identity exploration, relationship challenges, or experiences of oppression. It’s essential to be open and honest during the session, as your responses will help inform the next steps in your treatment plan



What Happens During the Intake Session?

While each session is shaped by your unique story, the process usually includes:


1. Intake Forms Before your appointment, you’ll complete our New Client Paperwork via our online Patient Portal. These forms will cover your medical and mental health history, family background, current medications, and other important information.


2. Welcoming and Discussion As the start of your appointment, your clinician will introduce themselves, share more about our approach to inclusive psychotherapy, and invite you to discuss what brings you in. Some common questions might include:

  • What concerns or goals are motivating you to seek therapy now?

  • Are there specific symptoms, like anxiety, sadness, or stress, impacting your daily life?

  • How do your intersecting identities—race, culture, gender, sexuality, and more—shape your experiences?


3. Assessment Process Your clinician may use evidence-based screening tools to get a clearer picture of your mental health. They will also explore your coping strategies, strengths, and support systems.


4. Feedback and Next Steps By the end of this first session, you’ll receive feedback on initial impressions and recommendations. Together, you will discuss possible next steps, which could include:

  • Scheduling follow-up therapy sessions

  • Exploring specific therapy modalities (such as couple's therapy, family therapy, EMDR, etc.)

  • Considering referrals for medication evaluation if needed


How Can You Prepare for Your First Session?

A little preparation can make your first session more comfortable and meaningful. Here are a few suggestions:

  • Reflect on Your Concerns: Take some time to think about what you’d like help with. Writing down your thoughts can be helpful.

  • Identify Significant Events: Consider any major life events or transitions that have impacted you.

  • Gather Relevant Information: If you’ve had prior mental health care, bring along any records you’d like to share.

  • Be Yourself: Our therapists are here to hold space for your story without judgment. Honesty and openness will help us collaborate effectively.


By taking these steps, you will feel more empowered and equipped for a constructive dialogue during your intake session.


What to Expect After the Intake Session

It’s common to feel a mix of relief, hope, or uncertainty after your first session. All of these feelings are valid.


Here are some things you can expect:

  • Personalized Action Plan: You’ll leave with clarity on recommended next steps tailored to your needs.

  • Follow-Up Appointments: We may schedule your next therapy session or discuss further assessment.

  • Resources and Self-Care: Your clinician may share immediate tools—like grounding exercises, reading materials, or support groups.
